CHD Offers Service Coordination

United Neighborhood Centers of Northeastern Pennsylvania’s (UNC) Community Health Department recently received approval as a Service Coordination and Transition Services Entity for Lackawanna and Luzerne Counties through the Pennsylvania Office of Long-Term Living.

UNC’s Community Health Department is approved to provide service coordination for the following medical assistance waiver programs:

• Attendant Care/ACT 150 Program – Provides services to eligible persons between 18-59 with physical disabilities, allowing them to remain as independent as possible

• Aging Waiver – Provides services to eligible persons over the age of 60, allowing them to receive in-home supports to remain as independent as possible

• Community Care Waiver (COMMCARE) – Designed for individuals with traumatic brain injury (TBI) over the age of 21, to allow them to remain as independent as possible

• Nursing Home Transition Coordination – Offers support and guidance from a service coordinator, connection to home and community-based services, and other supports, such as transportation, personal assistance, peer support, assistive technology, and home modifications

Anyone who uses Medical Assistance who may need help coordinating their own healthcare services or for someone else can contact UNC for information and support. They can also list ‘United Neighborhood Centers of Northeastern Pennsylvania’ as the Service Provider for Service Coordination on the Home and Community Based Services Waiver Application.

For those interested in using UNC as a service provider, each patient will receive an Individualized Service Plan (ISP), which will be developed at UNC’s initial assessment, detailing the specific services and needs the patient has. UNC will work with the patient, their caregiver(s), and other service providers in the community to draft and implement this plan, ensuring that the patient receives the best care possible.
