Foster Care Transitional Housing Program

In collaboration with Lackawanna County Children and Youth Services, UNC created the Foster Care Transitional Housing Program, which is geared for those youth exiting foster care who may have needs for additional support as they move into adulthood and independence. This pilot program will provide the needed safety net and supportive system to assist this special group of youth to become strong, independent and self-sufficient adult community members. UNC will provide two youth with an intensive mixture of housing and supportive services that will assist these most vulnerable youth to grow into adulthood with success. Services provided will include:

  • Scattered Site Supportive Housing
  • Job Skills & Training
  • Intensive Case Management
  • Life Skills
  • Financial Literacy
  • Job Training & Job Coaching
  • Educational Support
  • Mental Health Support
  • Drug & Alcohol Support
  • Mentoring
  • Communication and Relationship Skill Development
  • Permanent Housing Placement
  • Follow-Up for six months
