Case Manager – Permanent Supportive Housing

Position Summary:

Reporting to the Director of Community Services, the Permanent Supportive Housing Case Manager provides supportive services to homeless families, assists to find suitable housing, and provides life skills support to participants. A strong candidate must possess comfort with excel, data entry, customer service in the form of in person and electronic correspondence.

The Permanent Supportive Housing Case Manager must possess strong interpersonal skills, ability to work with diverse populations, and good communications skills.

Responsibilities:

Provide and coordinate services, as follows:

  • Provide housing case management to a case load of homeless families.
  • Recruit and educate landlords about the Permanent Supportive Housing Program, and the benefits of participation.
  • Have a thorough understanding of HUD regulations related to this position.
  • Conduct intake processes, assessing the needs of individuals and families who walk in for our services.
  • Create a monthly budget and a detailed action plan with all clients.
  • Assist in finding safe and affordable housing in the community, based on the needs of the participant.
  • Help participants to read and understand lease agreements.
  • Maintain accurate client files, financial documentation, and record keeping, including entering comprehensive client information into the ClientTrack database.
  • Connect participants with employment services and/or workforce development programs.
  • Assist consumers with connecting to medical/mental health supports.
  • Link participants with various supports with the school districts or childcare options.
  • Conduct home and office visits with consumers.
  • Provide program reports.
  • Coordinate efforts with other UNC programs, including referrals.
  • Enter service and program information into ClientTrack client management system.
  • Any other duties deemed necessary by Community Services Director. Other duties as assigned.

Education/Experience/Licensure

  • High school diploma or equivalent is required.
  • Bachelors degree in Human Services, Business Administration or related field is preferred.
  • Proficiency in MS Office 365 is preferred.
