Case Manager – Permanent Supportive Program

 

REQUIRED:   Bachelors Degree in Social Work w/experience working with homeless individuals and/or those with disabilities.

 

The ideal candidate is someone who is a team player, flexible, and detail-oriented with demonstrated experience balancing competing priorities and assisting those with housing needs such as evictions, homelessness and budgeting skills.

Responsibilities:

Provide case management for chronically homeless individuals in Permanent Supportive

Program as follows:

  • Assess the needs of homeless individuals referred by agencies that provide emergency housing for homeless persons.
  • Contract with the individuals in setting goals that work toward self-sufficiency, tracking both the needs and the systems available to meet those needs.
  • Work with program participants to meet goals and objectives. Facilitate group meetings of individuals in the program. Evaluate both the individuals’ progress and services provided.
  • Refer clients to other agencies and work with those agencies to treat the client with a holistic approach
  • Work with clients to develop life skills necessary in order to achieve self sufficiency
