Workforce Case Manager

Preferred:   Bachelors Degree in Social Work w/experience working with homeless individuals and/or those with disabilities.


Provide case management for participants in supportive housing programs as follows:

  • Assess the needs of homeless individuals or others referred by agencies that provide emergency housing services.
  • Meet regularly with program participants to help them set and achieve goals that work toward self-sufficiency, connecting them with needed supports and resources to help them successfully complete the program.
  • Work with landlords to find appropriate housing for participants and ensure that housing is safe and maintained, and that communication is clear.
  • Complete paperwork associated with housing, including paying rents and utilities on time.
  • Evaluate both the individuals’ progress and services provided
  • Conduct intakes for the housing counseling program and teach budgeting classes as needed. Conduct general intakes for unscheduled appointments.
  • Maintain all necessary record keeping, including entries in ClientTrack, the agency’s Homeless Management Information System.
